In order to confirm your attendance in the Women's Health Science Program, please complete this on-line registration form by no later than Monday, June 17th. If you are under the age of 18, a parent or guardian must help you complete the registration form as their signature is required on several documents such as the Northwestern University Laboratory Visitor Waiver and Photography Release Form. 

We encourage WHSP students to get to know one another, so you will be asked to provide some brief information about yourself and a recent photo which can be shared in the course packet.

The Women's Health Science Program will be taking photos and/or video during the Oncofertility Science Academy. Images or video may be used in on our website, social media, press releases, printed publicity and other content created by the Women's Health Science Program.
